United States First-Time / In-Person Passport Application (Form DS-11)

Application for a U.S. passport book and/or card using Department of State Form DS-11, the form used when an applicant must apply in person — first-time applicants, applicants under 16, and anyone whose most recent passport was lost, stolen, damaged, issued more than 15 years ago, or issued before age 16. This document describes the applicant inputs and required evidence and ends at the acceptance-facility handoff: DS-11 must be signed in person before an acceptance agent and is not mailed in. It does not submit the form; the live travel.state.gov source is always authoritative (see verification).
